Ontario Perception of Care (OPOC) Survey Participation Information

Are you a current client of our Mental Health or Transitional Aged Youth (TAY) program or have been a client within the past 6 months? We really value your opinion on how you feel about the services and supports that you have received from our organization and would love to hear from you!

Starting November, 2018, stop by one of our Youthab offices to complete the Ontario Perception of Care (OPOC) survey and help us improve our services! *Please be sure to bring ID*


As a client in the Mental Health, Transitional Aged Youth (TAY), Transition Home or Supportive Housing program, we really value your opinion on how you feel about the services and supports that you have received from our organization. We would very much appreciate it if you could tell us about your experiences by completing the OPOC survey.

Why is this important?

Your feedback will help us understand how we can improve the quality of our services and care for you and others. This is also an opportunity for our funders (the government) to hear from clients, which will help to improve mental health and addiction services across Ontario.

Is my participation voluntary?

Completing the survey is voluntary. You do not have to answer every question if you are uncomfortable doing so, you can stop the survey completely at any time. Choosing to stop or not participate in the survey will not affect the treatment or support you receive in any way.

Is it confidential?

This survey is completely confidential and data collected will not be connected in any way to you personally. Your name or any personal identifiers (e.g. client number) will not be included anywhere on the survey. All responses will be securely stored on password protected computers and paper copies will be kept in a locked cabinet which is only accessible by relevant staff. All staff are committed to maintaining your confidentiality and ensuring that the data collected in this survey remain anonymous and that no personal information or personal health information is shared. Please do NOT use any names (your name, your therapist’s name) or identifying information in any of the responses.

What will be involved?

You have a choice of completing the survey online or the paper version. For the online version, you will be given a computer generated code to enter to start the survey. A support person will be available if you need help with completing the survey.
There are (32 to 38) questions related to the program, followed by 12 questions about you (e.g. age, gender, language). It takes most people between 10 to 15 minutes to complete this survey. Not all items will apply to everyone - if a question does not apply to you, please check the “Not applicable” box - instead of leaving the answer blank.
Questions 27 to 29 only apply to clients who are nearing completion or have been discharged from the program. For those who are NOT nearing completion of services, please select “Not applicable”.

How is the information I provide going to be used?

Your responses will be combined with responses from many other people in a province-wide database maintained by the Centre for Addiction and Mental health (CAMH). The combined information will be shared with our organization, the Local Health Integration Network (LHIN) in your area, and the Ministry of Health and Long-Term Care to provide important feedback about our services. The combined information will also be included in reports generated by CAMH which are designed to help enhance mental health and addiction services across Ontario. Again, your individual answers will not be identifiable.
